Dear Applicant:
Thank you for your interest in the PLuS Program at DePaul University.
Please be aware that admission to the PLuS Program is separate from admission to DePaul University. You must be accepted by DePaul University to be eligible for PLuS services. Please review carefully the following forms, and complete the ones that are relevant to your situation. Upon receipt of your completed forms, your eligibility for the program will be considered.

To access the forms, click on the links underneath the description of each form. Microsoft Word will open the requested file. If asked if you want to open or save the file, choose to save it in your computer. After saving the file (make sure you remember where you saved it), open the file and you can either type your answers or print it and complete the form; all files are in Microsoft Word format.
Application Forms:
- Psychoeducational evaluation
Send to PLuS a copy of your most recent psychoeducational evaluation which has a diagnosis of a Learning Disability and/or Attention Deficit Disorder. This evaluation should be no more than five years old for LD, or three years old for AD/HD. It should include measures of intelligence (such as the WAIS-III), measures of academic performance (such as the WJ-III, WIAT-II, etc) and measures of information processing. This might be the most important document you send to PLuS; without this document PLuS cannot provide any services or accommodations to you. -
Verification of Attention Deficit Disorder
If you have been diagnosed with Attention Deficit Disorder, and if the initial diagnosis is older than three years old, forward this ADHD Verification Form to the professional who made or who can verify the diagnosis.

Case History Form
The Case History Form has been developed by the PLuS Program and its purpose is to inform PLuS of your strengths and weaknesses. It is of the utmost importance that you complete this form accurately, thoroughly and honestly. The clearer a picture of who you are PLuS has, the easier it will be to provide you with what you might need to succeed at DePaul.
- Freshman students should fill out this Case History Form.
- Transfer students should fill out this Case History Form.

Permission to contact Professional
In the event that the PLuS Program needs further information or clarifications regarding your strengths and weaknesses and/or your diagnosis, or if you want the PLuS Program to contact any professionals who worked or are working with you (LD specialists, psychologists, psychiatrists, etc.) please complete, sign and return this Permission to Contact Professional Form to PLuS.

Release of Information Letter, and Professional's Report
If you want any professionals who worked or are working with you (LD specialists, psychologists, psychiatrists, etc.) to contact and/or mail information to PLuS, please sign the Release of Information Form and forward it to them along with the report form. Use one pair of these forms for each professional.

Student Records Consent Statement
This Student Records Consent Statement outlines what you need to do if you decide to take the DePaul placement tests with accommodations, and it gives permission to PLuS to contact Student Records on your behalf. It also provides information and directs you to the Office for Students with Disabilities (OSD) to discuss accommodations for a disability other than LD and/or AD/HD.
