If you would like to have weekly meetings with a PLuS Clinician (LD specialist), you will need to complete one or more of the following forms.
Please make sure that you have indicated the quarter for which the fee will apply, and if you would like to be a part-time (once a week) or a full-time (twice a week) student with PLuS.
