It is strongly suggested that every course syllabus include a statement addressing the responsibilities of faculty and staff in relation to students with disabilities.
Elements for Syllabus Statement
A syllabus statement is included to provide:
- An invitation to students who have disabilities to meet, in a confidential environment, to review course requirements and discuss their need for accommodations.
- Information regarding the appropriate university offices who handle the records of students with disabilities
- A reminder to make accommodation requests in a timely manner. It is, however, necessary to remain flexible.
You may create your own statement or you may use the following sample statement:
Sample Syllabus Disability Statement
Students who feel they may need an accommodation based on the impact of a disability should contact me privately to discuss their specific needs. All discussions will remain confidential.
To ensure that you receive the most appropriate reasonable accommodation based on your needs, contact me as early as possible in the quarter (preferably within the first week of class), and make sure that you have contacted the:
- PLuS Program (for LD, AD/HD) at 773-325-1677, Student Center #370, and/or
- The Office for Students with Disabilities (for all other disabilities) at 773-325-1677, Student Center #370
