One of the roles of the PLuS Program is to be aware of the academic progress of the students participating in the program. The purpose of this form is to help each student set realistic goals based on objective information provided by her/his professors. The information you provide will guide both the student and PLuS in how to organize the remainder of the quarter.

Please note that:

• The student has given permission to PLuS to contact you regarding his/her academic progress
• All information will be shared with the student
• The submitted data is stored in a secured, password-protected server
