Forms
The PLuS forms are listed alphabetically. Some of them are password protected and will only allow you to complete missing fields or follow links. Saving them first in your computer will result in easier navigation. Use the 'Tab' key to quickly move in between fields. If you need to complete a form, you may:
- complete it electronically, and e-mail it back to PLuS
- print it, complete it and mail it, fax it, or deliver it to PLuS
When you download a form:
- if asked, choose to save the file in your computer
- remember where in your computer the file is saved
- open the file and complete the form
- use 'Save As...' to save the completed form and give it a new name
