Any group that receives $1,500.00 or more from the Cultural Center Allocation Fund is required to have audience members survey the quality of the program. An audience evaluation form has been provided for your convenience. Completed surveys should be returned to the Cultural Center within two weeks of program. 

The following form should be used when sponsored programs feature a guest speaker or presenter.
